This study was performed to evaluate the prognostic significance of initial muscle fiber type grouping in patients with acute necrotizing myopathy (ANM). Immunohistochemical analysis was performed in 15 ANM patients who had been diagnosed with ANM based on a typical pattern of necrotic fibers on muscle biopsy specimens. As a control group, 20 patients with noninflammatory myopathy (NIM) were also analyzed. At the time of muscle biopsy, the ANM group included 8 patients with acute necrotizing polymyositis (PNP), 5 with dermatomyositis (DM) and 2 with inclusion body myositis (IBM). The other 7 patients with ANM were diagnosed with DM. During the follow-up period, 7 (46.7%) of the 15 ANM patients died, including 3 patients with ANM-DM, 3 with ANM-IBM, and 1 with ANM-PNP.
